Qualifications for obtaining a Medical Marijuana Authorization:



"Qualified Medical Diagnosis"


Cancer, HIV, AIDS, Multiple Sclerosis, Epilepsy, Seizures, Chronic Pain, Spasticity Disorder(s), Chronic Pain, Nausea, Muscle Spasms Migraine Headaches, Hepatitis , Crohn's Disease, Glaucoma, Arthritis, Asthma, PTSD, Terminal or debilitating medical condition, Intractable pain.

Diseases, including Anorexia, which result in nausea, wasting, appetite loss, cramping, seizures, vomiting, muscle spasms, or spasticity, when these symptoms are unrelieved by standard treatments of medications.



You will need a valid or current Washington Drivers License,

Military Photo I.D. or a Washington State Photo I.D.



Current Medical Records with the "diagnosis" of your "qualifying diagnosis"

We need to have your medical records within 14 days of your appointment if you were unable to obtain them prior to your appointment.  After that time, your authorization will be suspended until we receive the records.




Minors will need to be accompanied by a parent or guardian.  The parent will be required to obtain a a "letter of acknowledgement" from the "primary physician".


Authorizations are for One (1) year.

You will need to "renew"your authorization every year.
